What is case management?

Case management is a collaborative process that typically involves a patient having a dedicated case manager (usually someone from allied health) that assists with the coordination of health assessments, planning, facilitating care, evaluation and advocacy for options and services. Your case manager is there to enable a seamless healthcare experience from start to finish and make the process easier for you. Their tasks will vary according to your unique circumstances, but your case manager can assist with both physical and lifestyle challenges that arise as a result of your pain – from coordinating assessments and care on your behalf to assisting with problems such as an inability to work due to pain.

 

When should I consider a case manager?

Case management is best suited to patients who have multiple medical, psychiatric and/or social conditions in association with their chronic pain – particularly if these conditions require the assistance of multiple doctors and other health care providers. Having a case manager to help navigate the challenges that arise can not only alleviate potential stress, but it can also promote your recovery process and ensure all the differing areas are coordinated in a safe and effective way.

 

What is involved in the process?

Anodyne can work with you in determining whether a case manager is the right addition to your pain management health care plan. As part of your initial assessment, we will conduct a goal setting plan. Based on these findings a case management plan can then be put in place. Sometimes this initial assessment can be completed by your doctor, but also by one of our experienced allied health care team members.
